At this stage, we would like to provide you with a copy of our welcome pack. We’ve also included a leaflet which will help to explain who can get treatment under the NHS if you are under 18 years old.

Steps to Book Your Consultation

These are the three steps you need to take before seeing us for a face-to face consultation:
  • Pre-Assessment Questionnaire – This is the medical history questionnaire which is compulsory before any face-to-face appointment is booked.
  • Provide us with your NHS number – It is now mandatory that your NHS Number is provided if you are under 18 years age. This a 10 digit number found on the NHS App or via your GP.

Once you have completed the above information, please do contact us 10 days later, allowing us time to update your records, and we will book the in-surgery appointment at an appropriate time.

  • Choice of treatment location with NO waiting time to start your treatment
  • Options for virtual consultations to significantly reduce the number of practice visits
